Thank you for responding to our survey and sending through your thoughts about our health and social care integration plans and programme.  We are currently in the process of updating and resetting our programme so that we can take into account our learning and some changes in focus due to the ongoing COVID-19 pandemic.  We will look at the views provided through the survey as part of this exercise and post a fuller response in September.

Overview

Partners across the NHS in East Sussex, East Sussex County Council, Healthwatch East Sussex and the voluntary and community sector have been working together to draw up a Health and Social Care Plan for our county. The draft plan sets out the health and social care needs of our East Sussex residents, and how we intend to drive the transformation needed to meet them over the next 3-5 years.

Through our new joint plan we want to continue our work to improve our population’s health and wellbeing and reduce health inequalities, by breaking down barriers between health and social care to deliver the right care and support, at the right time, in the right places for local people.

The draft plan focuses on our agreed priorities and next steps for transformation and integration between health and social care; prevention, children and young people, community services, urgent care, planned care and mental health.

There is growing demand on both NHS and social care services. More and more local people will require support and care for long term conditions. By joining up the care we provide we will be better able to support people to live as independently as possible and achieve the best possible outcomes for them.

Over the next few weeks the draft East Sussex Health and Social Care plan will go through each of our partner’s formal sign off processes.

Our East Sussex plans have been informed by our most recent engagement with local people and our stakeholders. Our conversations with local people and our stakeholders will be ongoing as we start to deliver the plan together. As part of this, there is an opportunity to share your comments on the draft East Sussex Health and Social Care Plan through an online feedback form. Your feedback and ideas will be used to inform how we take forward work on our priorities in the coming year.

The Sussex Health and Care Strategy

Our East Sussex plan also forms part of a wider health and social care strategy for the whole of Sussex. The Sussex-wide strategy is a response to the national NHS Long Term Plan and covers joint priorities for East Sussex, West Sussex and Brighton and Hove, as well as Sussex-wide clinical priorities. For example maternity services, cancer, stroke, falls and fractures.
