NHS Health Checks Client Survey
Overview
How helpful did you find your NHS health check?
If you've recently had an NHS health check, we want to hear about your experience of it and any changes you might have made to your lifestyle as a result.
Why your views matter
An NHS health check is a 'Health MOT' that aims to see what risk a person has of having heart disease, a stroke, diabetes, and kidney disease. The health check is available every five years to those aged 40 to 74, who don't already have one of these conditions. As part of the check, people receive personalised advice on lowering any health risks and maintaining a healthy lifestyle.
We would like to know how well the NHS health checks are helping people to feel more informed about their health and make positive change. This will help us understand what works well with the health check, and what could be improved to better support people.
The survey will take roughly 5-10 minutes to complete and you have the option to use the ‘Save and come back later…’ button at the end of each page if you need more time to finish later. If you would like to see the questions before you start you can find them in the preview section below.
If you need the survey in an alternative format, printed and posted to you, in large print or on coloured paper, please get in touch with us.
