Survey for carers of people with Severe Mental Illness

Overview

All GPs are expected to provide a physical health check every year for patients who are registered as having a severe or serious mental illness (SMI). This is because they are more likely to experience poor physical health on average compared with the general population.

This includes people who have a diagnosis of schizophrenia or bipolar disorder, or who have a history of psychosis. Attending an annual physical health check can help with prevention and early identification of health conditions, and link people with community organisations.

This survey is intended for people who live anywhere in Sussex who give support or care to a person with a SMI diagnosis. By completing this survey, you could contribute to improvements to the way health and care services are delivered.

The supporting document is a template for a letter for GP surgeries in Sussex to use when inviting people for their physical health check, which includes a second page giving further information.
